OFFICIAL RULES — Hamilton's Mushroom Vault Giveaway
NO PURCHASE OR PAYMENT IS NECESSARY TO ENTER OR WIN. A PURCHASE OR PAYMENT WILL NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED.
1. Sponsor
The Hamilton's Mushroom Vault Giveaway ("Sweepstakes") is sponsored by Hamilton's Mushrooms, LLC ("Sponsor"), 1552 Greystone Drive, Carbondale, Colorado 81623, United States.
2. Eligibility
The Sweepstakes is open only to legal residents of the fifty (50) United States and the District of Columbia who are eighteen (18) years of age or older at time of entry. Employees, officers, directors, contractors, and members of the immediate families (parent, child, sibling, spouse) and households (whether related or not) of Sponsor and its affiliates, distributors, and wholesale partners are not eligible to enter or win. Void in Puerto Rico, all other US territories, and wherever prohibited or restricted by law.
3. Sweepstakes Period
The Sweepstakes begins May 12, 2026, at 9:00 AM Mountain Time and ends July 10, 2026, at 11:59 PM Mountain Time ("Sweepstakes Period"). Sponsor's computer is the official timekeeping device for the Sweepstakes.
4. How to Enter
To enter, complete and submit the Hamilton's Mushrooms 2026 Customer Survey at https://form.jotform.com/Pevec/win-the-hamiltons-mushroom-vault during the Sweepstakes Period and check the box indicating you wish to enter the drawing, providing the requested information (first name, last name, email address, state of residence, and the eligibility confirmation). Submission of a complete survey with the drawing-entry box checked constitutes entry. Limit one (1) entry per person and per email address. Multiple entries from the same person or email address will be void; only the first valid entry will be considered.
There is no other method of entry. No purchase is required. Internet access is required to enter.
5. Prizes
A total of five (5) prizes will be awarded:
Grand Prize (one winner): "A Year of Hamilton's" — one (1) 100g bag of each of the following Hamilton's Mushrooms products: Body Blend, Mind Mix, Lucid Lion's Mane, Crucial Cordyceps, Reishi Rampart, True Chaga; one (1) 1 fl oz Reset tincture; one (1) 1 fl oz Agarikon tincture; one (1) Hamilton's branded t-shirt (your choice of available in-stock size); and one (1) Hamilton's sticker pack. Approximate retail value (ARV): $378.94.
Runner-Up Prizes (four winners): "The Hamilton's Sampler" — winner's choice of any two (2) 100g Hamilton's Mushrooms products; one (1) Hamilton's sticker pack; one (1) Hamilton's branded t-shirt (your choice of available in-stock size). Approximate retail value (ARV): $122.98 each.
Total ARV of all prizes: $870.86.
Prizes are non-transferable. No cash equivalent or substitution will be made except by Sponsor in its sole discretion, which reserves the right to substitute a prize of equal or greater value if any advertised prize becomes unavailable. All taxes, fees, and surcharges associated with the receipt or use of any prize are the sole responsibility of the winner.
6. Drawing
On July 13, 2026, Sponsor will randomly select five (5) winners from all eligible entries received during the Sweepstakes Period. Drawing will be conducted using a verifiably random selection method (random.org's True Random List Randomizer or equivalent). Sponsor will document the random selection process with a timestamped record. Odds of winning depend on the total number of eligible entries received.
7. Winner Notification
Winners will be notified by email at the address provided at entry, on or about July 13, 2026. Each winner must respond to the notification email within seven (7) days of the notification date to claim their prize and provide a valid US shipping address. If a winner cannot be reached, fails to respond within the response period, is found to be ineligible, or otherwise fails to comply with these Official Rules, the prize will be forfeited and an alternate winner may be selected from the remaining eligible entries.
Names of winners (first name and state of residence only) will be posted at hamiltonsmushrooms.com/pages/giveaway-winners-2026 on or about July 13, 2026. Winners list will remain available for at least sixty (60) days after the drawing date.
8. Prize Delivery
Prizes will be shipped to confirmed winners within thirty (30) days of confirmation of the winner's shipping address, via standard US shipping at Sponsor's expense. Sponsor is not responsible for prizes lost, stolen, or damaged in transit.
9. General Conditions
By participating, entrants agree to be bound by these Official Rules and the decisions of the Sponsor, which are final and binding in all matters. Sponsor reserves the right, in its sole discretion, to disqualify any entrant found to be tampering with the entry process or the operation of the Sweepstakes; violating these Official Rules; or acting in an unsportsmanlike or disruptive manner. Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify, or suspend the Sweepstakes should any virus, bug, unauthorized intervention, fraud, technical failure, or other cause beyond its control corrupt the administration, security, fairness, or integrity of the Sweepstakes.
10. Releases
By accepting a prize, each winner releases Sponsor and its parent, subsidiaries, affiliates, distributors, and the directors, officers, employees, and agents of each from any and all liability for any loss, harm, damages, cost, or expense arising out of participation in the Sweepstakes, the acceptance, use, or misuse of any prize, or any claim based on publicity rights, defamation, or invasion of privacy.
11. Publicity
Except where prohibited by law, by accepting a prize each winner grants Sponsor a non-exclusive, royalty-free, perpetual, worldwide license to use winner's first name, state of residence, photograph, voice, and biographical information for advertising, promotional, and publicity purposes in any media now known or hereafter devised, without further notice, compensation, or approval, and waives any claim to additional compensation for such use.
12. Privacy
Personal information provided by entrants will be used by Sponsor for the purpose of conducting the Sweepstakes (entrant verification, winner selection, winner notification, prize fulfillment) and as otherwise described in Sponsor's Privacy Policy at hamiltonsmushrooms.com/policies/privacy-policy. Entrants who opt in to Sponsor's email marketing list at the time of entry may receive marketing communications from Sponsor and may opt out at any time. Entry into the Sweepstakes does not require email marketing opt-in.
13. Disputes
Except where prohibited, each entrant agrees that any and all disputes, claims, and causes of action arising out of or connected with the Sweepstakes or any prize awarded shall be resolved individually, without resort to any form of class action; any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred but in no event attorneys' fees; and under no circumstances will any entrant be permitted to obtain awards for, and entrant hereby waives all rights to claim, indirect, punitive, incidental, or consequential damages, or any other damages, including attorneys' fees, other than entrant's actual out-of-pocket expenses (i.e., costs associated with entering the Sweepstakes), and entrant further waives all rights to have damages multiplied or otherwise increased.
All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules, or the rights and obligations of the entrant and Sponsor in connection with the Sweepstakes, shall be governed by, and construed in accordance with, the laws of the State of Colorado, without giving effect to any choice of law or conflict of law rules.
14. Tax Information
The Approximate Retail Value of the Grand Prize is $378.94 and each Runner-Up Prize is $122.98. No prize exceeds $600 ARV; therefore Sponsor will not issue an IRS Form 1099-MISC. Winners are nonetheless responsible for any applicable federal, state, and local taxes on the prize received.
15. Severability
If any provision of these Official Rules is held to be invalid or unenforceable, such provision shall be struck and the remaining provisions shall be enforced.
16. Winners List Request
A list of winners (first name and state only) will be available at hamiltonsmushrooms.com/pages/giveaway-winners-2026 on or about July 13, 2026, and remain available for at least sixty (60) days. To request a copy of the Official Rules or the winners list by mail, send a self-addressed stamped envelope to: Hamilton's Mushrooms, LLC, Attn: Mushroom Vault Giveaway, 1552 Greystone Drive, Carbondale, CO 81623, to be received no later than September 10, 2026.
